  2. Emergency Contacts: You authorize us to contact your designated emergency contact in case of a medical or safety concern. 

  3. Communication Preferences: Phone Messages: Limited information may be left on voicemail unless otherwise instructed. Patient Portal: Secure communication is available via the patient portal for non-urgent matters. SMS/Text Messaging: Appointment reminders and general notifications may be sent via SMS. By signing, you consent to receive such messages unless opted out in writing.

  4. Right to Access and Amend: You may request access to or amendment of your medical records. Requests should be submitted in writing.

  5. Privacy Rights: You have the right to restrict disclosures, request confidential communications, and file complaints if you believe your privacy rights have been violated.

  6. Data Protection: Your PHI is stored and transmitted securely per legal requirements.

No mobile information will be shared with third parties/affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third-parties. End users can opt out of receiving further messages by replying STOP or ask for more information by replying HELP.
